Items of concern and questions
A written list in certain situations is a priceless tool. Utilize our Appointment Form (See: “General Forms” section) to help you remember.
If you or your client have more than a few items to discuss or questions to ask, prioritize them to be certain that you will talk about the most important ones first.
For example

Are there any symptoms from the new medication you just started to tell the doctor about?

Has an issue become more difficult to deal with? If so, make a list of symptoms or changes.

Does anything make it better or worse? When does it occur? Does it affect my daily activities?
Be factual and be honest!
Before going to the doctor, make a prioritized list of concerns and questions to discuss.
