Prescription changes
If any new or changed prescriptions have been received by the individual, please be sure to use the pharmacy the individual is already using.
The customary pharmacy will have the individual’s medical history and be able to tell if the possibility exists of a drug interaction with other medications.
New medications and changes in existing medications should be carefully documented, in the Medication Record before leaving from this encounter to be sure this important document is always current.
Recap appointment concerns, questions and answers
Please take a few minutes to go over the questions and answers which took place at the appointment to be certain the individuals has a good understanding of the issues and any prescribed treatments or changes.
Write a brief written description of the entire appointment (include areas of all questions and answers, issues, treatments and changes). Tangible documentation of what went on at the appointment may reinforce again and again what the doctor said.
Talking with the family or friends
If the client has given you permission to speak with designated family or friends, please do so after clarifying the issue to be discussed.
The client has the full decision making upon what information is to be shared with designated family or friends.
Remember the client must agree to convey any new tests, treatments, or diagnosis, along with any new appointments for the individual.
The individual maintains the right to make all decisions, whether you feel they are right or wrong.
